What is Postural Hypotension?

Postural hypotension (also known as orthostatic hypotension) is a very common and debilitating condition, caused by a drop in blood pressure (BP) on standing up. It can cause symptoms such as dizziness, falls and fainting. It affects up to one in five older people, reduces their quality of life and increases their risk of hospitalisation and dementia. 

Why does it Occur?

  •  Medications: Over 250 medications have been linked to postural hypotension, especially treatments for high blood pressure which are commonly prescribe in this population.
  • Nervous System Issues: When the autonomic nervous system doesn't regulate blood pressure effectively, it can cause problems adjusting to position changes.
  • Age: The body's ability to adapt to these changes can decrease with age.
  • Medical Conditions: Diabetes, Parkinson's disease, and certain heart issues can interfere with blood pressure control.

What is the current treatment and why is it unsuccessful?

There has been very little research on how best to treat postural hypotension. Due to the nature of the condition, medications are not the best option.  Individuals with orthostatic hypotension (OH) have expressed a preference for non-drug treatments; however, adherence to available options such as compression garments, increased fluid intake, and physical maneuvers is often poor. Numerous published medical papers describe the clinical challenges associated with adherence to non-drug therapies, which remain the primary treatment hurdle within this population.
